Forms Packet If you are a Mississippi Resident and will be receiving (having transferred) your class 3 item directly from Donald Ray Cresswell - these are the blank forms you will need to use. These forms are customized to already have some of my information filled in on them. Note, if you are buying as an "Individual" you will still need to contact me to receive a set of fingerprint cards (can not be scanned or emailed - must use originals as furnished by ATF). This form packet contains the same forms as included in the above packets. |
